Community Liaison

Job Summary: The purpose of this job is to communicate, network, build strong relationships, educate and follow-up with referral sources in the Cuyahoga & Summit county areas. This position requires a great deal of time to be spent in the community and hospital environment. The Community Liaison will be responsible for organizing and implementing referral development, marketing and assessment programs to assure awareness and a positive community image. Must be able to work independently, set and track goals and develop an effective community outreach program.

Requirements:

  • RN, RPN, or Social Work designation with registration in good standing.
  • Strong networking, presentation, and follow-up skills.
  • Excellent communication & listening skills.
  • Self confident, enthusiastic, strong initiative and motivation, and a desire to succeed.
  • Ability to work independently and handle multiple tasks concurrently.
  • Ideal candidate to have hospital and/or home care experience.
    Marketing/Sales experience is highly preferred.

Responsibilities include:

  • Visit, revisit, inform and educate between 25 and 30 network referral sources on a weekly schedule.
  • Conduct a minimum of 2 educational presentations/info sessions to groups each month to raise awareness.
  • Assist R-DAYs Home Care Services in developing, managing and maintaining referral relationships.
  • Acknowledge all network referral source contacts with appropriate correspondence.
  • Continually research and identify sources for client referrals.
  • Maintain records & provide the Executive Director with weekly networking activities and progress report.
  • Provide advice to management on key developments within the service area.
  • Keep up to date on information and issues related to the position, the community, referral networks, and the company.
  • Participate in internal and external professional development activities as appropriate.

Work Hours:

The position is flexible and will require the candidate to work independently approximately 16 hours per week.

Caregiver

Place of Employment:  Client homes, work sites outside of the client home as required and other work sites as required by R-DAY’s Home Care Services management.

Accountable to: R-DAY’s Home Care Services Director, Administrator, Manager, Supervisor or Nurse-in-Charge.

Job Summary: The Caregiver is a front-line position acting as a representative R-DAY’s Home Care Services and providing the services offered by the company.  The Caregiver duties are not limited to those listed as the position has a wide scope of duties at times dependent on the client and always under the auspices of the company.  The Caregiver will mostly work independently of co-workers or supervisors and therefore must balance good judgement and initiative along with the policies of the company. 

It should be noted that emphasis is for the Caregiver to involve the client in the care program to help provide the client with the basic standards of healthy living as follows:

  • Physical Activity
  • Mental Stimulation
  • Personal Contact
  • Nutrition
  • Relaxation
  • Humour

Work Hours: This position may require various shifts at various times (including nights, weekends, and sleep-over) in one or more work sites.

Duties:  The following list of duties is a guideline of possible duties to be performed independently or as a group of duties in the course of working with a client.  The Caregiver duties are not limited to those listed.

  1. Companionship: One of the most important aspects of R-DAY’s Home Care Services will be providing customers with a strong sense of self-esteem and independence.  Taking a genuine interest in the client, their life and their environment will help create true sense of companionship.  Whether it is playing a favourite board game or card game, reading, taking a stroll or visiting with friends and neighbours, companionship provides the depth of our services.
  2. Housekeeping, Laundry, and Pet Care: The CAREGIVER will provide dusting, washing hardwood and flooring, vacuuming carpets and furniture, changing bedding, scrubbing the bathroom; pre-washing, washing, drying, ironing and folding laundry; defrosting refrigerators/freezers; cleaning ovens/stoves and china cabinets; caring for house plants; taking out garbage and general tidying of living spaces as required or requested. It also includes caring for pets including grooming, feeding, exercising, walking, playing, cleaning up after the pet.
  3. Kitchen Work: Grocery shopping, preparing menus and meals, cooking, baking, serving, assisting with eating, washing and drying dishes and general tidying up is included.
  4. Transportation and Escort: A CAREGIVER may drive their car and transport a client to an appointment or errand if the CAREGIVER has written authorization from the company to do so.  The CAREGIVER may also escort the client to appointments or errands providing a helping hand or shoulder to lean on.

Qualifications: 

  1. Current certificate in standard first aid and CPR.
  2. Valid driver’s licence and reliable transportation
  3. Ability to work independently.
  4. Excellent communication and listening skills.
  5. Compassionate, patient and caring nature.
  6. Able to work in a stressful environment.
  7. Must be physically and mentally fit to handle the demands of care giving.

General Notes: 

The Caregiver will be required to maintain a safe working environment for themselves and their clients. 

There may be emergency situations where the Caregiver will have to react quickly with sound judgement.

LPN/LVN

Place of Employment:  Client homes, work sites outside of the client home as required and other work sites as required R-DAY’s Home Care Services management.

Accountable to R-DAY’s Home Care Services Director, Administrator, Manager, Supervisor or Nurse-in-Charge.

Job Summary: The LPN/LVN is a front-line position acting as a representative R-DAY’s Home Care Services and providing the services offered by the company.  The LPN/LVN duties are not limited to those listed as the position has a wide scope of duties at times dependent on the client and always under the auspices of the company.  The LPN/LVN will mostly work independently of co-workers or supervisors and therefore must balance good judgement and initiative along with the policies of the company.  It is the responsibility of a Licensed Practical Nurse/License Vocational Nurse, under the supervision of the RN Supervisor, Unit Manager, Assistant Director of Nursing, Director of Nursing and the Administrator, to perform routine treatments and services in the delivery of care to the residents, patients.  An LPN/LVN must serve as a staff nurse, and at times as a charge nurse on the floor and is responsible for the daily work assignments of the nursing assistants.  An LPN/LVN will provide information for evaluation of nursing assistants concerning performance appraisals and in educating and listening to family and residents’ concerns.

  1. Prepares and administers medications and treatments with knowledge of purpose and adverse effects of nursing care provided.
  2. Completes all documentation for observation, planning, and evaluation of care provided to residents, including orders by physicians.
  3. Functions in monitoring capacity as assigned for completion of resident’s care, employee guidance and discipline.
  4. Maintains open lines of communication between nursing shifts, as well as other departments, supervisor, physicians, and families of residents.
  5. Acts on residents behalf to assure patient rights and to prevent resident abuse.
  6. Acts on first aid knowledge in emergencies.
  7. Is aware of safety issues, accomplishing responsibilities in a safe manner, re-enforces good safety practices displayed by other staff members.
  8. Is able to maintain confidentiality of information.
  9. Works as a team member.
  10. Is able to perform the following physical activities: Kneeling, Stooping, Standing, Reaching, Pulling/pushing, Walking, Grasping, Moving fingers, Hearing, Talking, Sitting, Repetitive motions.

Qualifications: 

  1. Current certificate in standard first aid and CPR.
  2. Current LPN or LVN license in this state or permit to practice.
  3. Valid driver’s licence and reliable transportation
  4. Ability to work independently.

Office Assistant/Secretary

Place of Employment: Office of R-DAYs Home Care Services (RDHC) or approved remote location.

Accountable to: R-DAYs Administrative Director, Manager, and Supervisor.

Job Summary:  As the first point of contact and representative of R-DAYs Home Care Services, the office assistant/secretary plays a vital role in R-DAYs day-to-day operations.

NOTE: The office assistant/secretary does/will not play a direct role in any activities involving direct care services to clients.

Work Hours: This position may require various shifts and/or times depending on workload. No shift/times outside of regular office hours will be required or expected.

Duties: The following list of duties is a guideline of possible duties to be performed independently or in collaboration with other R-DAYs staff. The office assistant/secretary duties are not limited to those listed.

  1. Answers phone – forwards calls – takes messages
  2. Filing and creating files
  3. Labeling/stamping documents
  4. In coming/Out-going mail (mailing letters/marketing materials – receiving/picking up incoming mail).
  5. Printing/copying/faxing documents
  6. Researching
  7. Monitoring office cash/petty cash
  8. Accounts receivable (incoming checks/paid invoices via mail, only)
  9. Make Bank deposits

RN

Place of Employment:  Client homes, work sites outside of the client home as required and other work sites as required R-DAY’s Home Care Services management.

Accountable to R-DAY’s Director, Administrator, Manager, Supervisor or Nurse-in-Charge.

Job Summary: The LPN/LVN is a front-line position acting as a representative R-DAY’s Home Care Services and providing the services offered by the company.  The LPN/LVN duties are not limited to those listed as the position has a wide scope of duties at times dependent on the client and always under the auspices of the company.  The LPN/LVN will mostly work independently of co-workers or supervisors and therefore must balance good judgement and initiative along with the policies of the company It is the responsibility of a staff Registered Nurse under the supervision of a RN Manager, RN Supervisor, RN Assistant Director of Nursing, RN Director of Nursing, and Administrator to perform routine treatments and services in the delivery of care to the residents.  A RN must serve as a staff nurse, and at times, a charge nurse on the floor and irresponsible for daily work assignments of the LPN’s, LVN’s and nursing assistants.  An RN will provide information for the valuations of nursing assistants and LPN’s concerning performance appraisals and in educating, consoling, or listening to families and clients.

It should be noted that emphasis is for the RN to involve the client in the care program to help provide the client with the basic standards of healthy living as follows:

  1. Is aware of areas that make residents subject to abuse and prevents such abuse.
  2. Maintains open lines of communication among the nursing shifts, and departments, supervisors, and families.
  3. Is aware of safety issues and strives to accomplish all responsibilities in a safe manner, consistently reinforces good safety attitudes and practices displayed by other staff members.
  4. Controls, prepares, and administer treatments and medications with knowledge of purpose and adverse effects of the nursing care provided.
  5. Functions as a knowledgeable and supportive committee member upon request by the Director of Nursing to maintain high levels of health care.
  6. Complete all documentation for assessments, observations, planning evaluation, orders by physicians and care provided to clients.
  7. Utilize professional assessment skills to identify physical, mental, and psychosocial needs of residents, patients and is able to develop a care plan to meet these needs in a multi-disciplinary approach.
  8. Functions in a supervisor capacity as assigned for the completion of a residents and patients care, employee’s guidelines and disciplines, and maintenance of a positive environment.
  9. Functions as a resource person and mentor to peer and subordinates to maintain professional nursing standards.
  10. Acts on clients behalf to assure patient rights and is aware of and prevents residents’ abuse.

Qualifications: 

  1. Current certificate in standard first aid and CPR.
  2. Current RN license in this state or permit to practice.
  3. Able to supervise (instruct, assign work, etc) Nursing Assistants, LPV’s, and LVN’s.
  4. Valid driver’s licence and reliable transportation
  5. Ability to work independently.
  6. Excellent communication and listening skills.
  7. Compassionate, patient and caring nature.
  8. Able to work in a stressful environment.
  9. Must be physically and mentally fit to handle the demands of care giving.
